How we verify

Mammography Index is compiled from public records, not from who paid to be listed.

Sources

• FDA MQSA certified facility list (public.zip) — Every facility currently certified under the Mammography Quality Standards Act: name, address, phone and fax. Published and updated weekly by the U.S. Food and Drug Administration; searchable on FDA's certified facility search.
• CMS Outpatient Imaging Efficiency – Hospital (wkfw-kthe), measure OP-39 — Breast cancer screening recall rate for each Medicare-certified hospital's outpatient department: the share of Medicare screening mammograms followed by diagnostic imaging within 45 days. Published by the Centers for Medicare & Medicaid Services.

We download FDA's MQSA certified-facility file every week and list every facility in a U.S. state, DC or territory; military facilities with APO/FPO addresses are left out. That gives about 9,100 facilities. Every one held a current FDA certificate on the file date shown on its profile.

We match facilities to Medicare-certified hospitals in CMS's Outpatient Imaging Efficiency file only when the facility's phone number is the hospital's, or its ZIP code, street number and street name match the hospital's address, in the same state. For matched facilities we show the hospital's breast cancer screening recall rate (CMS measure OP-39) and the national median across hospitals with a reported rate. OP-39 covers Medicare fee-for-service outpatients of the hospital as a whole; it is not a rating of an individual facility or doctor.

Facilities are labelled mobile when their name or address contains words such as mobile, coach or van, and hospital-based when they match a CMS hospital or their name says hospital or medical center. FDA's file does not say which services a facility offers.

Order within a city: facilities with a published Medicare recall rate first, then other fixed sites, then mobile units, then alphabetical. This is not a quality ranking. Featured facilities appear in a separate, labelled box, pay a flat monthly fee, and never change that order.

What a badge means

FDA-certified (MQSA): The facility appears in FDA's list of facilities certified under the Mammography Quality Standards Act (the MQSA public facility file, updated weekly by FDA) on the date shown. We only publish positive matches. A listing without a badge is not an accusation — most gaps are name variations between registries.

Freshness

Data was last refreshed on 2026-10-16. FDA updates its certified-facility list weekly and CMS updates hospital data quarterly; we re-pull both weekly. Confirm a facility's certificate with FDA before your appointment.
